Complex Evaluation (up to 90 minutes): $200
Standard Evaluation (up to 60 minutes): $125

 Evaluation: Aiming to build a foundation for ongoing treatments or provide a functional diagnosis, an evaluation consists of subjective/verbal review of symptoms, hands-on assessment of movement and function, specific tests for involved body parts/current symptoms, education for self care, development of a treatment plan, and referral if necessary. Evaluation sessions include initial treatment, frequently including an initial home program.

Most evaluations will be scheduled for 60 minutes, however if you have multiple concerns or expect extensice treatment at your first visit, we ask that you schedule a 90 minute evaluation. 

Treatment: Treatment sessions are 30 or 50 minutes in length and can only be scheduled if one of the above services has already been performed. These include ongoing treatment and education based on the plan created during the consultation/evaluation/assessment and are specialized for each client. ​

All payments are due at time of service. We do not bill insurance, however we can provide an invoice if you would like to request reimbursement from your insurance carrier.
